- State License # (if not already affixed on prescription)
Submitting Prescriptions:
Fax the prescription from your office to our pharmacy at 800-531-2390.
Call our pharmacy at 800-898-8026. Mon-Fri, 8am-5pm CST. Feel free to leave a confidential voice mail message for our pharmacy after hours.
Mail the prescription to us at the address above. Alternately, your client may mail the original prescription to us.
